Parents/Guardians wanting Parent Teacher interviews please complete this form and click the "send info" button. The form will be sent to Mrs. Bell in the Counselling Office (724-3284 ext 6). The deadline for requests is March 19th. Confirmation of your request will be emailed to the address entered in the form.

If you have already been contacted by phone to indicate that you have appointments scheduled please do not return this form.

Telephone appointments will be taken on March 22nd only, by calling 724-3284 ext 230. Written and email requests will have first priority.
